year, most employment paperwork will be submitted electronically. Once we receive your application and have been
contacted by Dave Waskiewicz to confirm your employment, you will receive two emails to complete your
employment paperwork. The employment information
must be complete prior to starting work.
1)
Background
check. You will receive an email from
Kim Coggins requesting that you fill out an electronic consent form for a
background check. We will check criminal
and driver’s records, and sex offender registries. Please submit this information within five (5)
days of receiving this email.
2)
Employment
Paperwork. ADP is our new payroll
services provider. You will receive an
email from ADP requesting that you download the ADP app, set up a username and
password. Once logged in, you will be
prompted to go through the employment setup, including providing your personal
information, filling out tax forms, employment eligibility verification (I-9
Form), and direct deposit. You will also
be prompted to upload a copy of your social security card and driver’s license
(or student ID). All your pay information will be
assessable to you through the ADP app.
Work permits continue to be required for those under the age
of 18.

The Club has filled in the employer section. Take the completed form to your child’s school office to complete the school section. The form can also be completed by the administrator at Northport Public Schools. Please call 231-386-5153 or email [email protected] to make an appointment .
The permit must be turned in prior to the first day of camp.
